Welcome to Rutland House Surgery

We are pleased to accept registration requests from patients who are living within the area indicated on the map below:

New Registrations

We welcome new patients and If you would like to join Rutland House Surgery and live within our catchment area (please call the reception team to double check) Please complete the attached form(s) and bring them along with one proof of ID and address. One of our reception member would happy to help you to complete your registration process.

If you have a NHS card, please bring this with you. We will need your NHS number in order to validate your registration via our computer system. If you do not have a card, please contact your previous GP to obtain your NHS number; this would process your registration faster.

Please ask reception member for online access if you prefer to book your appointment online and you can also request your repeat prescription.

Please fill in the GMS1 form with either the adult or child questionnaire.

 

Online Registration

If you would like to register online, please see below links for each form to complete online. Please come in to the surgery after you have filled in the forms online so that we can verify the documents you submitted. Please note that we will not do the registration until you come in to the practice in person. We will hold on to the registration form and the documents you submitted up to 4 weeks. If we don’t hear from you we will assume you are no longer interested to register at our practice.

  • Please fill in both GMS1 and Adult/Child questionnaire
  • Children’s questionnaire should be filled in for ages between 0-16 years
  • Use Adult questionnaire if you are over 16 years of age
  • Please attach Birth Certificate If you are registering your child who is under the age of 5
  • If child is born abroad, please provide Birth certificate and record of all immunisations given
